No. If a person does not belong to the SEBC, the ICR Survey Form shall NOT be filled up in respect of that person/ family.
No. The Enumerator shall fill up the ICR Survey Form using Black Ball Point pen only.
No. The Enumerator shall fill up the information using the Numerals and Block Capital Letters in the style as indicated at the top of the ICR Survey Form.
The list of District Code, Block/ ULB Code, GP/ ULB Ward Code, Village Code etc. will be provided to the Enumerator in the Enumerator’s Kit.
After entering the “First Name” with one letter (Block Capital) in a box Leave a box blank  then enter the "Middle Name" with one letter (Block Capital) in each box  after entering the “Middle Name” leave a box blank  then enter the "Surname" with one letter (Block Capital) in a box.
No. For example, if the EPIC Number is OR/01/007/049170, then the EPIC Number should be entered as OR01007049170.

The Date of Birth shall be filled up in the Date-Month-Year format. For example, if the Date of Birth is 1st Feb, 2010 then it will be entered as 01-02-2010 and if the Date of Birth is 28th Dec 1972, it will be entered 28-12-1972.

When the Date of Birth is not mentioned in the identity proof of the person, then Age of the person as stated by him/ her shall be entered in the assigned boxes.

In case a family has more than five members (Head of Family + 4 family members), the Enumerator shall fill the details of the rest of the family members using additional ICR Survey Form(s) repeating the information of the Head of Family along with District, Block/ULB, GP/ ULB Ward, Village code; Ration Card Number, Mobile Number and Class code in additional form(s). For example, 2 (Two) ICR Survey Forms shall be used for families with 6 to 9 (Nine) members; if there are up to 13 (Thirteen) members, then 3 (three) ICR Survey Forms shall be used and so on, depending on the total number of members of the Family.
If the left thumb of the surveyed person is missing, then the impression of the right thumb should be taken. If both thumbs are missing, the impression of one of the fingers of the left hand starting from the forefinger should be taken. If there are no fingers on the left hand, the impression of one of the fingers of the right hand starting from the forefinger should be taken. If no finger is available, then the Enumerator shall obtain the Signature/LTI of any other family member.
No. The Enumerator shall pack the filled in ICR Survey Forms using the envelopes provided to them in the Enumerator’s Kit only, as use of any other envelope may cause damage to the edges and corners of ICR Survey Forms, rendering them unfit for data retrieval.
No. The enumerator cannot use eraser, pen, correction-fluid etc. or over write to rectify any wrong entries in the ICR Survey Form. In such cases, the Enumerator shall use another form. However, it is always advisable that the Enumerator should be sure of the information before filling up the same in the ICR Survey Forms.
Yes. The Enumerator shall be provided with an Identity Card issued by the concerned BDO/ Executive Officer of Urban Local Body/ Commissioner, Municipal Corporation.
In case, there is any family member/s not belonging to SEBC by birth whereas the Head of the Family belongs to SEBC, the details of such family member/s shall not be entered in ICR Survey Form.
Where the Head of Family does not belong to SEBC but any other member of that family belongs to SEBC, details of such member shall be entered in the ICR Survey Forms. In this case, details of such SEBC member shall be entered as the Head of the family. If more than one member of such non-SEBC family belong to SEBC, in that case the eldest SEBC member will be entered as the Head of the Family and details of other SEBC member(s) shall be entered in the same ICR Survey Form.
